Camden Diabetes Integrated Provider Unit (IPU)
The Camden IPU is a multi-trust partnership set up to meet the needs of people with diabetes in Camden by taking a different approach to diabetes care. CNWL NHS Trust is the lead provider, working in partnership with CHP, The Royal Free London and University College London Hospitals NHS Trusts. We work together to ensure patients are seen at the right time, in the right place, by healthcare teams equipped with the appropriate skillset. CHP provides diabetes nurse specialists, podiatry and psychology support.
The team helps patients with complex health issues who need multiple therapies. The aim is to avoid hospital attendances and prevent risk of long-term complications. Patients are provided with the skills, knowledge and confidence to self-manage their diabetes.
The IPU provides:
- Virtual consultant advice and guidance
- Access to a specialist diabetes nurse
- Psychological assessment/intervention
- Advice from a dietitian
- Bengali education in diabetes
- Desmond education sessions
- Advice and guidance
Initial and follow-up appointments will be made by the service’s administration team at Peckwater Health Centre for clinics at that site or at Belsize Priory Health Centre.
